Environmental Health Forms and Guidelines

Approval from The City of Greater Geraldton’s Environmental Health Team is required for a range of activities related to the community’s health and safety.  To obtain approval you will need to submit the relevant application form along with any associated fees.

To lodge an application please download and complete the relevant form.  You will need to submit your application along with the payment before it can be assessed by a Health Officer.

You can lodge the form in person at: Geraldton Civic Centre – 63 Cathedral Avenue, Geraldton.  Payment in person can be made via cash, EPTPOS, cheque, money order or credit card at the Customer Service Desk Monday - Friday 8.30am to 4.45pm.

If no payment is required – Foodstall Notification for Charity ONLY – applications can be emailed to council@cgg.wa.gov.au
If sending a cheque or money order please send, along with completed application, to City of Greater Geraldton PO BOX 101 Geraldton WA 6530.
